After submitting your Registration Form, select your desired plan(s), and submit all payments to our preferred source of payments: Registration fee, class fee, and add-ons via,

LIABILITY & INJURY DISCLAIMER
Everlasting Talent is not responsible for any items lost, damaged, stolen, and/or any physical injuries. Everlasting Talent is not responsible for any lack of skill, achievement, and/or non-exposure of talent. EverLasting Talent has the right to decline services due to inappropriate disciplinary conduct with nonrefundable fees. EverLasting Talent has the right to charge fees if the student remains beyond their appointment time of $5 per 15 minutes. Everlasting Talent is not a talent agency or agent company.
I understand that all published media productions participated in are voluntary and for training purposes, and that I am not anticipating or expecting any form of compensation. I agree to waive all media rights including photos, videos, music, recordings, filming, productions, graphic edits, used during classes and or events, and release my rights for marketing purposes to EverLasting Talent. Everlasting Talent has the right to revise the policy, prices, schedules, calendars, and fees without notice.
WARNING: Under Georgia law, there is no liability for an injury or death of an individual entering these premises if such injury or death results from the inherent risks of contracting COVID-19 or any ailments. You are assuming this risk by entering these premises.
RESCHEDULES, CANCELLATIONS & REFUND DISCLAIMER
Requests to reschedule a lesson must be submitted by student in writing at least 24 hours in advance. If approved, the rescheduled lesson must be redeemed within 7 days, and is subject to studio and instructor's availability and is not guaranteed. Discount pricing is for 4 consecutive booked lessons. Cancelled classes, and/ or missed appointments, are non-refundable and are not guaranteed a make-up lesson. Pay-as-you-go service is an available option and may be applied due to inconsistant attendance.
I agree that I am /over the age of 18 and authorize the student named to register for classes with Everlasting Talent. I have read and agree to the class terms and registration policy listed therein. I do understand that registration, class tuition and all applicable fees due must be pre-paid in full to confirm enrollment. I further understand that all fees are non-refundable are subject to change at any time. Thank you in advance
